If a student is absent for any reason, this form must be completed by a parent/guardian. We ask that it be completed by 8:30 am on the day of the absence to allow our administrative team time to properly account for all students. It may even be completed in advance when planned absences are upcoming.

ALL Grade Levels: Please submit excuses for tardies (such as Dr. notes) to Sarah Wallack, our Registrar, at sarah.wallack@burtonacademy.org. Please remember that for all absences, the Student Absence Reporting Form must be completed at the link above.
